This session focuses on innovative approaches in addressing organizational silos in data collection, assessments, and reporting.

Making Community Health Implementation Plans (CHIP) more Accessible and Robust to Community Stakeholders: CHIP Chat: Every three years, non-profit hospitals are required to conduct a community health needs assessment (CHNA) and implementation plan (CHIP) in order to maintain their non-profit status which includes annual reporting and updates on strategies and goals from previous reports. Oftentimes, hospitals may find themselves searching for data across various platforms, making the annual reporting tedious and laborious. This session discusses how the utilization of dashboard platforms (e.g. Tableau, PowerBi) can streamline the CHIP process, and be highly effective in communicating data for the said process while also making data more accessible.

Reimagining the Collection of the Member Voice: This presentation will focus on the process and outcomes of the 2022 Health First Colorado Member Survey, including results on access to care and improvement strategies. Attendees will be educated on how to engage interdepartmental stakeholders, incorporate member participation within survey construction, and report results on members' experience. Attendees will be encouraged to collect the member voice through innovative methods that improve partnerships, dismantle data silos between organizations and communities, and involve equitable methods to understanding community voice.
